An exciting opportunity within Practice Plus IUC as an Advanced Clinical Practitioner within the new Clinical Resilience Team, providing enhanced clinical assessment and decision-making for patients accessing urgent care services. You will use telephone, video and online consultations to assess, diagnose and agree safe onward management plans, ensuring patients receive the most appropriate care without unnecessary repeat assessment.
Working within the Clinical Assessment Service and alongside NHS 111 and Out of Hours teams, you will play a vital role in delivering high-quality, evidence-based care 24 hours a day, seven days a week. You will work autonomously within your scope of practice while contributing to a collaborative multidisciplinary environment.
This is an opportunity to make a real difference to patient outcomes by providing timely clinical advice, prescribing where appropriate and supporting colleagues across urgent care services. Your expertise will help maintain safe, effective and responsive services for patients of all ages.
You will also support service quality, clinical governance and the development of innovative models of care, helping Practice Plus IUC deliver outstanding patient-centred services.
